The Be Prepared Kit was prepared by State of Ohio agencies responsible for disability and aging services and Ohio's Developmental Disabilities Network. Using the Be Prepared Kit, you will complete several checklists. The checklists will guide you on the documents you need to copy and keep and the supplies you should have on hand at all times. When you have completed the checklists, keep them together in a waterproof folder.
The Be Prepared Kit is offered in Web and PDF formats. If you are downloading the PDF version for printing in booklet format, you may need to change your printer settings to 11 x 17 page size.
If you print the PDF version, lines are printed on the paper to guide you on how to fold the card to a smaller size.
